What your notice must include

Counter-notice
If you believe your content was removed in error, you may submit a counter-notice to the same agent including your signature, identification of the removed material and its prior location, a statement under penalty of perjury that the removal was a mistake, and your consent to jurisdiction in Delaware.
Repeat infringers
We may terminate, in appropriate circumstances, the accounts of users who are repeat infringers.
